 California State Requirement for students entering 7th grade
for the 2012-13 school year.

To help protect your children and others from whooping cough, a California law now requires students to be vaccinated against whooping cough. 
The Tdap vaccine is required for all 7th grade students.
You must bring your students' vaccine records to the school for proof of Tdap immunization.

MPUSD asks that you provide your student's updated shot record by May 15, 2012.  Students will not be admitted to school in August 2012, without having provided proof of Tdap immunization.
